These Terms of Service (“Terms”) govern your access to and use of the Hart Pharmacy website and any forms, links, or features made available through it (the “Site”). By using the Site you agree to these Terms. If you do not agree, please do not use the Site.

2. Medical disclaimer

Information on this Site is provided for general educational purposes only and is not medical advice, diagnosis, or treatment. It does not create a pharmacist-patient or provider-patient relationship. Always consult your pharmacist, physician, or other qualified provider before starting, stopping, or changing any medication, supplement, or therapy.

Never use this Site for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. For suspected poisoning, call Poison Control at 1-800-222-1222.

3. Use of our online forms

Contact, prescription transfer, and immunization request forms are not monitored around the clock and are not a substitute for speaking with a pharmacist. Submitting a form does not guarantee that a prescription will be transferred, filled, or that an appointment has been confirmed — a team member will follow up with you.

4. Prescriptions and compounded medications

All prescription medications, including compounded preparations, require a valid prescription from a licensed prescriber. Compounded medications are not FDA-approved products; they are prepared for an individual patient based on a prescriber’s order. Product availability, pricing, and turnaround times may change without notice.

5. Acceptable use

You agree not to:

• Submit false, misleading, or another person’s information without authority.
• Attempt to access accounts, data, or systems you are not authorized to use.
• Interfere with, disrupt, or place undue load on the Site.
• Use the Site for any unlawful purpose or to obtain medication unlawfully.

6. Third-party links and services

The Site links to third-party services such as our patient portal, telehealth partners, scheduling tools, and mapping providers. We do not control those services and are not responsible for their content, availability, or privacy practices. Your use of them is governed by their own terms.

7. Intellectual property

The Hart Pharmacy name, logo, text, images, and layout on this Site are owned by Hart Pharmacy or its licensors and may not be copied or reused without written permission. Other marks referenced on the Site belong to their respective owners.

These Terms are governed by the laws of the State of Kansas, without regard to its conflict-of-law rules. Any dispute will be brought in the state or federal courts located in Sedgwick County, Kansas.
